To me, it was the first real 'emotion' shared by both characters. Sammy knew he had screwed with Dean too long, and he took a beating to prove he was sorry (pup didn't raise a finger to defend himself, and we all know he could have). And Dean, he bought in to saving Sammy at that moment. He put the knife down, and did what he had to do in order to clear the air.
While Sam may not 'feel' what he did was wrong, he showed that Dean was still important enough to him that he would allow himself to be hurt to make Dean feel better.
And Dean, he cleared the slate.
They aren't back to an even keel by a long shot, but that felt like the first honest exchange they'd had since the beginning of the season.
